Cutaneous and Mucosal Lichen Planus: A Comprehensive Review of Clinical Subtypes, Risk Factors, Diagnosis, and Prognosis

چکیده
Lichen planus (LP) is a chronic inflammatory disorder that most often affects middle-aged adults. LP can involve the skin or mucous membranes including the oral, vulvovaginal, esophageal, laryngeal, and conjunctival mucosa. It has different variants based on the morphology of the lesions and the site of involvement. The literature suggests that certain presentations of the disease such as esophageal or ophthalmological involvement are underdiagnosed. The burden of the disease is higher in some variants including hypertrophic LP and erosive oral LP, which may have a more chronic pattern. LP can significantly affect the quality of life of patients as well. Drugs or contact allergens can cause lichenoid reactions as the main differential diagnosis of LP. LP is a T-cell mediated immunologic disease but the responsible antigen remains unidentified. In this paper, we review the history, epidemiology, and clinical subtypes of LP. We also review the histopathologic aspects of the disease, differential diagnoses, immunopathogenesis, and the clinical and genetic correlations.

Oral lichen planus (OLP) is a relatively common mucosal disease that can present isolated or associated with cutaneous lichen planus. Contrarily to its cutaneous counterpart, though, OLP tends to be chronic, relapsing, and difficult to treat. Severe morbidity is related to erosive forms, and more aggressive presentations have been described, such as the "gingivo-vulvar syndrome". This article r...

متن کاملOral Lichen Planus or Oral Lichenoid Reaction? A Literature Review
Background and Aim: Oral lichen planus (OLP) is a chronic, inflammatory, T-cell-mediated autoimmune oral mucosal disease. Oral lichenoid lesions develop as a type IV hypersensitivity reaction. Both of these entities are potential precancerous conditions; this adds to their clinical significance. The purpose of this literature review was to detect the similarities and the differences of these le...
متن کاملLichen Planus – A Review
Lichen planus is a fairly distinctive, chronic, immunologically mediated mucocutaneous disease, of uncertain etiology that was first described as a disease of the skin that can also affect mucosal surfaces, including those that line the oral cavity. The prevalence of LP in the general population is of the order of 0.9 to 1.2%. It is stated that approximately 40% of lesions occur on both oral an...
متن کاملQualitative and quantitative study of immune cells in various types of cutaneous lichen planus
Background: Lichen planus (LP) is a common inflammatory skin disease. Pathogenesis of LP includes two possible mechanisms; related or unrelated to antigen. Regarding different clinical features of cutaneous and mucosal types of LP, for the first time, we decided to perform a qualitative and quantitative study of immune cells in different types of cutaneous LP and in comparison with normal skin....
